Position Summary

Will be responsible for linking and coordinating all aspects of care across the care continuum. Provides expert social work service to Adirondack Health patients, Medical Home, Outpatient settings. Facilitates psychosocial assessment and continuing care. Works collaboratively with physicians, care coordinators, nurses and other members of the multidisciplinary team and government and community agencies to effect positive patient and family outcomes. Self-directed. Serves as a patient advocate. May be required to work during weekends or evenings as patient needs dictate. May be required to travel to sites beyond the main campus.

Qualifications/Requirements

Bachelor’s degree in social work from an accredited school of Social Work preferred. Experience in the hospital setting preferred. Knowledge of community resources, health care financial and payer issues and eligibility for state, local and federal programs expected. Demonstrates critical thinking and problem-solving skills. Demonstrates advanced oral and written communication skills. Collaboration, negotiation and mediation skills; and time management and prioritization skills required. Familiarity with MS Word, Excel, Office and internet use required; familiarity with Meditech a plus and will be required within 3 months of hire

Salary Range:

$24.72 - $34.44 per hour
