Pediatric Navigator

Position Summary:

The Pediatric Navigator is an early child development expert who will join the pediatric primary care teams at the Tupper Lake and Saranac Lake Health Centers to provide interventions, referrals, and follow-up for families with patients primarily between the ages 0-3 years. The Pediatric Navigator builds strong relationships with families and providers to increase the efficiency of the medical system and support team-based comprehensive care.

 

Educational Requirements/ Qualifications:

  1. Master’s degree in psychology, social work, counseling, early childhood education, or related field highly preferred; Bachelor’s degree maybe considered if working towards Master’s degree; New York State license as appropriate.
  2. Clinically licensed mental health professional preferred
  3. Experience and knowledge about early childhood growth and development, parent-child relational health, infant and early childhood mental health, and family systems
  4. Ability to drive between clinic locations
  5. Excellent oral and written communication skills, experience with Word and Excel preferred.
  6. Commitment to working with underserved populations in a community setting
  7. Empathic, supportive, and patient
  8. Strong motivational interviewing skills
  9. Ability to work with people of many backgrounds and cultures
  10. Ability to take initiative and a willingness to learn
  11. Ability to work well in teams and independently
  12. Ability to collect and enter data for program management, evaluation, and reporting purposes.
  13. Interest in the use of technology to improve care quality a plus. Excel, Word, Outlook, etc.
  14. “Grace under pressure” with the ability to multitask and triage/juggle multiple appointments and conflicting priorities
  15. Comfort with an open-door policy, potentially frequent interruptions, and warm hand offs
  16. Flexibility in communication: able to switch between being on the floor playing with young patients and communicating with medical professionals regarding mental health interventions in a professional, coherent, and efficient manner
  17. Ability to tolerate isolation as may be the only behavioral health professional on site.
  18. “Professional backbone,” confidence, and assertiveness with providers and colleagues (shaping appropriate referrals, etc.)
  19. Reflectiveness, insight, and curiosity about oneself

Director of patient care services for outpatient clinics

 

Contacts:

Medical staff, nursing personnel, patients and their family members/caregivers, visitors, outside agencies and other Adirondack Health personnel
